"1261" bin iki yüz altmış bir sene-i hicriyesinde nefs-i İzmit'te tevellüd etmişimdir. Pederim İzmit'te Veli Hoca Mahallesi'nde altmış beş numaralı hanenin ikinci numarasında mukayyed çıkrıkçı esnafından merhûm Hasan Usta'dır. İbtidâ İzmit'te Sıbyan badehu Rüşdiye-i Mülkiye ve sonra İstanbul'da Şehzadebaşı'nda Mekteb-i Rüşdiye'de mukaddeme-i ulûmi bi't-tahsil "1276" bin iki yüz yetmiş altı tarihinde Mekteb-i Tıbbiye-i Askeriye-i Şâhâne'ye talebelik sıfatıyla kayd olmak şerefine nail oldum.
